Physician (Nursing Home) – Full-Time, Flexible Schedule

We are seeking an experienced Physician to provide comprehensive primary care in post-acute settings such as SNFs and ALFs.

  • Lead patient assessments, manage acute and chronic conditions, and coordinate care with teams and families.
  • Perform diagnostic testing, oversee clinical documentation, and contribute to quality initiatives.
  • Offer medical oversight, education, and collaborate with advanced practice providers.
  • Enjoy clinical autonomy, flexible scheduling, minimal on-call, and a supportive team environment.

    • Qualifications include an MD or DO with an active Minnesota license, board certification in Internal or Family Medicine, and 2+ years of geriatrics or primary care experience preferred.
