Seeking a Primary Care Physician for an innovative PACE program serving individuals 55+ with chronic illnesses or disabilities, promoting independence in their communities.

Key Responsibilities:

Provide high-quality primary care, assessments, and chronic/acute condition management

Develop and integrate personalized care plans with an interdisciplinary team

Conduct home and facility visits, participate in care planning and quality improvement

Support staff clinically and contribute to policy development

Requirements:

MD/DO with geriatrics interest, board certified or eligible in Internal Medicine, Family Practice, or Geriatrics

Current license, DEA registration, and minimum one year of elderly care experience
