We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY. This full-time position offers flexible scheduling (part-time or per diem options available), working Monday–Friday with occasional weekends/holidays.
- Conduct comprehensive assessments, develop treatment plans, and manage acute/chronic conditions in collaboration with physicians.
- Order and interpret diagnostic tests, prescribe medications, and ensure regulatory compliance.
- Participate in team meetings, coordinate care with interdisciplinary staff, and support nursing staff.
- Provide timely documentation and engage in quality improvement activities.
Qualifications include NY NP licensure, certification (AGNP/FNP), DEA registration, and 1-2 years of geriatrics or long-term care experience preferred.
