Hospice Nurse Practitioner

We are seeking a compassionate Hospice Nurse Practitioner to join our dedicated team in Greenfield, WI.

Provide holistic patient assessments and develop personalized care plans.

Administer medications and treatments as prescribed.

Collaborate with interdisciplinary teams to ensure quality outcomes.

Educate and support patients and families regarding hospice services.

Monitor patient progress and adjust care plans accordingly.

Maintain accurate documentation and participate in discharge planning.

Advocate for patient needs within the healthcare system.

Qualifications include a Master’s in Nursing, Wisconsin Nurse Practitioner license, and preferably hospice experience. Must have a valid driver’s license, be comfortable traveling, and possess strong clinical and communication skills.
