Nurse Practitioner – Pulmonary – SNF / LTC Rehab center

We are seeking an experienced APRN in pulmonary care for a part-time, in-person role in skilled nursing and long-term rehab settings.

Provide comprehensive pulmonary services, including assessments, diagnoses, and management of respiratory conditions like COPD, asthma, and pneumonia.

Manage noninvasive ventilation therapies and tracheostomy care, collaborating with multidisciplinary teams.

Perform admissions, discharge evaluations, and interpret diagnostics such as ABGs and imaging.

Educate patients and families, participate in care planning, and help reduce hospital readmissions.

Ensure compliance with regulations and document accurately in EMR/EHR systems.

**Qualifications:** Active APRN license, Master’s degree, 6+ months in long-term care, 3+ years nursing experience, BLS/ACLS, and ability to work independently.
