The Case Manager monitors and directs nursing staff to ensure resident care aligns with individual care plans, regulations, and quality standards.
Reports to the Director and Assistant Director of Nursing, supervises RNs, LPNs, TMAs, and CANs. Coordinates care with interdisciplinary teams, residents, families, physicians, and external providers. Performs RN House Supervisor duties, including physician rounds, assessments, and clinical oversight. Manages the MDS process, updates care plans, and participates in care conferences. Provides pain management, advocates for resident rights, and promotes continuous quality improvement. Ensures compliance with regulations, maintains professionalism, and fosters resident dignity and privacy. Requires RN licensure, nursing experience, and knowledge of regulations and documentation processes. Work hours: 40 hours/week, with holiday and weekend rotations.
Pay: $40.69 – $45.82/hr.
